Scheduling an appointment
If you are sick, injured, or are in need of a well check-up please call or stop in the Center to schedule an appointment. For sick visits or injury visits, we make every attempt to see you within 24 hours from when you contact us for an appointment. For well visits, physicals, women’s health, and immunizations, we try to schedule you within one week from when you present to us.
Confidentiality
The Health & Wellness Center has a very strict set of guidelines regarding confidentiality. We cannot release any information to your parents, another physician, or your friends without your expressed written consent. This means that we cannot even confirm or deny that you have scheduled an appointment. If you feel as though you would like us to be able to discuss your appointment with anyone, you must sign a release of information. The only exception to this policy is in the case of a life threatening injury or illness.
Obtaining medical records
A copy of your confidential medical records can be provided to your insurance, or sent to an employer, another university, or continuing care provider after you sign a release of information. A copy of your medical records will be provided to you within 5 business days from the receipt of the release of information. You may access the release form at Forms On-line or you may pick one up at the Health & Wellness Center.
Payment for Health Services
Payment for services can be made by charging your student account (your tuition bill) or by paying cash or check.
The Health & Wellness Center does not file claims to health insurance. Students will be provided a superbill receipt with specific codes for diagnosis and treatment to meet the requirements for most medical insurance carriers. If you have health insurance coverage, you may send a copy of the superbill to your insurance company for reimbursement.
